What website can I access to find more information on my diagnosis or symptoms?

SFC utilizes Up to date .com (http://www.uptodate.com/home) for the most current general medical information.  Click on the link for “Patients and Caregivers”, then you may search by symptoms, medical diagnosis.

How can I get a refill on my medication?

We ask that you first please contact your pharmacy so that they may send us a request. We also ask that you give us 24-48 hours to refill your medication. You may also send a refill request through the patient portal.
